WHAT IS TRANSITIONAL CARE?
Transitional care at Lakehouse provides short-term rehabilitation services for those that are looking to get “back on their feet” as quickly as possible. We provide care for medical challenges such as surgeries, stroke, or other serious medical conditions. Our seasoned team of professionals include physical, occupational, and speech therapists; along with social workers and dietitians who collaborate to develop a personalized care-plan for each resident. We monitor progress on a daily basis to ensure that we are doing our best to return your loved one to his or her former self.

TRANSITIONAL CARE FOR MEMORY CARE
At Lakehouse, we know that patience and understanding for those with memory disorders is an essential component of the rehabilitation success. Our transitional care for memory care program is one of only five in the state, and is designed to provide physical rehabilitation, while simultaneously providing emotional support and care for those with memory challenges. Many of our transitional care residents are home-based or based out of assisted living facilities, and come to Lakehouse to enjoy the unique transitional care experience we offer, while returning to their respective homes afterwards.
